Impact of silver diamine fluoride application on the oral health of older adults with loss of autonomy

Abstract

Introduction:


Dental caries is a major determinant of overall health and quality of life among older adults. Aging and loss of autonomy compromise oral hygiene, promoting the onset and progression of caries, which may lead to pain, infections, difficulties in chewing and speech, nutritional deterioration, and reduced overall well-being. This high prevalence also generates substantial costs for the healthcare system. Thirty-eight percent silver diamine fluoride (38% SDF) represents a promising therapeutic option for arresting the progression of active caries; however, its effectiveness among institutionalized older adults remains insufficiently documented.


Objectives:


• To evaluate the effectiveness of 38% SDF in arresting active caries among institutionalized older adults.

• To examine salivary biomarkers associated with inflammatory processes.


Methodology:


A 12-month randomized controlled pilot study is currently underway with 60 participants. The experimental group receives 38% SDF, while the control group receives a 0.9% saline solution at baseline (T0). Three data collection phases include dental examinations and saliva sampling. For ethical reasons, both groups receive two applications of 38% SDF.


Results:


•A significant slowdown in the progression of active caries is expected.

•Data on changes in dental surfaces and the cariogenic microbiota.

•Contribution to addressing the oral health needs of institutionalized older adults.


Conclusion and Implications:


38% SDF could become a simple, effective, and easily deployable therapeutic strategy to improve the oral health of frail older adults and reduce the burden of dental care in institutional settings. The findings may support the integration of 38% SDF as a public health strategy to enhance oral health and quality of life among vulnerable older adults.
