Importance of the leukoglycemic index as a prognostic marker of in-hospital evolution in patients with ST elevation acute myocardial infarction in HGZ N° 58

Abstract

Prognosis after ST-Segment Elevation Myocardial Infarction (STEMI) is related to the likelihood of adverse events and is multifactorial. Quiroga et al. were the first to propose the Leukocyte Glycaemic Index (LGI) as a prognostic indicator of death and in-hospital complications in patients with ST-Segment Elevation Myocardial Infarction (STEMI). Leukocytosis plays an important role in the process of onset and development of myocardial infarction.  The following formula is used to calculate ILG: ILG = (blood glucose [mg/dL] * leukocytes [106/l])/1,000 It was decided to carry out this study in a second level hospital, which has an influx of 6 family medicine units, the total population 500,000 people. It is an observational, descriptive, prospective, cross-sectional, descriptive study from September 2023 to February 2024. All cases of acute myocardial infarction were studied in order to evaluate patients who died and those referred to the third level.  When we categorised the leukoglycaemic index as greater or less than 1 as a reference value, we found that patients with in-hospital morbidity had a higher frequency of this category when compared to those without morbidity. Of the 100% of patients, 63% had an index greater than 1 and 37% less than 1. With a sensitivity of 75.8% of patients who really have a poor prognosis. Specificity of 55.6% giving an adequate sensitivity, but with low specificity for the objective of the article, we also found a value of 0.68 in the ROC curve, given the size of this sample we have an AUC without statistical significance with low sensitivity and specificity, so it should be reproduced with a larger sample to determine if it would have the expected impact for use in the Infarction Code.

Dafne Espinosa graduated in medicine from the Instituto Politecnico Nacional Mexico. She worked for the Star Medica Group and IMSS. Graduated as a specialist in emergency medicine from IMSS with her publication in the Association of Emergency and Disaster Medicine of Mexico. Currently working as a specialist in emergency room and intensive care.
