Title: Telehealth technologies effect on individual oral hygiene in children with end-stage chronic kidney disease

Abstract

The number of chronic kidney disease (CKD) children, especially those with end-stage kidney disease (ESKD), has been growing in recent years. ESKD associated with many oral and dentoalveolar manifestations. Patients undergoing dialysis and immunosuppressive therapy have poor oral hygiene. Today there is no system where dentists contact to CKD patients and their doctors such as pediatricians, nephrologists, urologists, transplantologists etc. This fact ultimately leads to decreased quality of patients’ life, high risk of odontogenic complications and adverse outcomes in CKD children. Our pilot study aimed to evaluate the impact of medical telecommunications between pediatric dentists, nephrologists and hospitalized CKD patients and telehealth technologies effect on individual oral hygiene in children with ESKD. 20 ESKD children participated in the study. Patients were divided into two groups: Group 1 – 10 children undergoing peritoneal dialysis or hemodialysis with an ICD-10 diagnosis «N18.0 End-stage kidney disease». Group 2 – 10 children undergoing immunosuppressive therapy with an ICD-10 diagnosis «T86.1 Kidney graft dysfunction». Oral hygiene index-simplified (OHI-S) was recorded for all participants before and after online dental healthcare education. All children showed a significant decrease in OHI-S level after teleducation. OHI-S values comparison in both studied groups showed that OHI-S in Group 1 significantly decreased after online-lessons by almost 2 times and more than 1.5 times in Group 2. Thus, medical telecommunications between pediatric dentists, nephrologists and hospitalized CKD patients significantly improve oral hygienic status in ESKD children.
