Postpartum cardiopulmonary deterioration and congenital pneumonia in the neonate: Clinical insights from a dual case
Abstract
The postpartum period is marked by rapid physiological changes that can exacerbate underlying conditions, particularly in women with comorbidities such as obesity. We report a case of a 31-year-old woman at 36 weeks of gestation with obesity and hypothyroidism, presented with mild respiratory symptoms and was found to have grade I diastolic dysfunction. She developed postpartum respiratory deterioration following cesarean delivery, along with progressive diastolic dysfunction and rising inflammatory markers. The late preterm neonate developed respiratory distress and was diagnosed with congenital pneumonia. Both mother and neonate recovered with timely supportive care. This case highlights the need for a predictive antepartum risk stratification model that integrates routinely available clinical parameters to identify high-risk mothers, guide delivery planning, and inform data-driven postpartum monitoring. It also underscores the importance of using antenatal risk factors to anticipate and prepare for early neonatal complications
